Why the home often gets missed
Every discharge plan assumes a home that can support it. Most of the time, nobody has looked.
Discharge plans are written for the patient, not the house
Case managers work under time pressure without training in home environments. Home health therapy starts after the person is already home. Contractors can build, but they do not assess how a person moves.
Falls are where the gap shows up first
About one in four adults age 65 and older falls each year, and falls lead to roughly one million hospital stays a year. Many start with a step, a threshold, or a trip to the bathroom in the dark.
Florida homes bring their own hazards
Sunken living rooms, pool decks, slider tracks, and garage entry steps. Then hurricane season, when a power outage becomes a medical event for anyone on oxygen or a powered bed.

What we do about it. We assess the person, the home, and the people who help, in person or by video. You get a photo report, a prioritized hazard list, and specific recommendations. When changes need a builder, you choose one and hire them directly. We never take money from contractors.

Talk with us
A 30 minute phone consultation about the person, the home, and your goals. $50, credited toward any visit you book within 30 days.
We assess the person and the home
In person, or by guided video before a hospital discharge. We look at how the person moves, not just at the house.
You get a written plan
A photo report, a prioritized hazard list, and recommendations, delivered to a secure portal, followed by a call to walk through it.
Changes get made right
We write exact specifications for the contractor you choose, then verify the finished work before anyone relies on it.
One clear answer before discharge day
Every Ready for Discharge report ends with a single readiness rating across ten areas of daily life, so everyone knows what has to happen first.
The rating describes the home's readiness for this person. It is not medical clearance.
We never take money from builders
Many home safety checks are done by the company that wants to sell you the renovation. We separate the two. We assess and prescribe. You choose from vetted, licensed contractors, and you contract with them directly.
No referral fees. No commissions. No payments in either direction.
