Every service starts with a $50 Initial Consultation. In 30 minutes by phone, Dr. L'HommeDieu learns about the person, the home, and your goals, recommends the right service, and schedules it with you. Your $50 is credited toward any visit you book within 30 days.

For now, we take card payments by phone: after you book, we call you to confirm the appointment and take payment.

Please keep health details out of the booking form. We only need your name, how to reach you, and the ZIP code of the home. We collect clinical information later, in our secure portal.

The Initial Consultation is a $50 conversation with Dr. L'HommeDieu that helps you decide what, if anything, you need next. It is the first step for every service.

What it is

A 30 minute phone call with Russ L'HommeDieu, DPT
A conversation about the person, the home, the people who help, and what worries you
A clear recommendation for the right next step, or an honest answer that you do not need one
Open to the person themselves, family members, and caregivers; more than one person can join
$50, credited toward any visit you book within 30 days
No obligation to book anything else

What it is not

Not a home assessment. We do not see the home, so there is no photo report, hazard list, or readiness rating. Those come with a visit.
Not a medical visit. There is no examination, diagnosis, treatment, or physical therapy, and it does not replace your doctor or therapist.
Not billed to insurance. It is a private-pay service and is not billed to Medicare or other insurance.
Not for urgent discharges. If someone is in the hospital or rehab now, book Ready for Discharge below instead.
Not for professionals referring patients. Physicians, therapists, discharge planners, and facilities can book a free partner call below.
Not for emergencies. If someone has fallen and is hurt, call 911.
