Ready for Discharge is a check of the home before a patient leaves the hospital or rehab, done personally by Dr. Russ L'HommeDieu, DPT, ATP, CFPS. It is for families and discharge planners who need to know whether the home can support the person on day one. It costs $250 in the home around Tampa Bay, or $175 by video for homes in Florida, New York and Ohio, and ends with one rating: Green, Yellow or Red.
Discharge readiness means whether the home, the equipment and the people helping are ready to support the patient safely from the first day home.

What happens
The $50 consultation
A family member or discharge planner books the call and gives the expected discharge date. Dr. L'HommeDieu schedules the discharge visit with you on that call.
Get the home ready
For a video visit, the family receives our Home Video Walkthrough Guide: a tape measure, good light and a charged phone are most of it. Forms are handled in our secure client portal.
The visit
About an hour. Dr. L'HommeDieu talks with the patient about their daily routine and concerns, then walks the home with the family: entry, the path to the bedroom, bathroom, kitchen and where equipment will go.
Report within 24 hours
The Discharge Home Readiness Report is posted to the portal, followed by a call to go over every Red and Yellow item.
Seven-day check-in
A short call after the patient is home: were the must-do items done, any falls or ER visits, anything new.

Common questions
Can it be done while my family member is still in the hospital?
Yes. We assess the home, not the hospital care. A family member can meet us at the home or walk us through it by video. With written permission, we can send the report to the discharge planner or case manager.
What if discharge is only a few days away?
Book the earliest consultation time you see. Dr. L'HommeDieu schedules the discharge visit on that call, and rush scheduling within 48 hours is available for $50.
Is the rating a medical clearance?
No. The rating describes whether the home can support this person. Medical decisions about discharge stay with the hospital team.
