Check the home before the patient leaves the hospital or rehab. One guided video session, a written report within 24 hours, and one clear rating.
How the session works
Book and confirm locations
A family member or discharge planner books and gives the expected discharge date. We confirm the patient and the home are both in Florida.
Sign forms in the secure portal
Telehealth consent, the Release Authorization, and permission to request therapy notes from the facility.
Get the home ready for the call
The family receives our Home Video Walkthrough Guide: a tape measure, good light, and a charged phone are most of it.
A three-way video session
About an hour. Dr. L'HommeDieu talks with the patient about their daily routine and concerns, then walks the home by video with the family: entry, path to the bedroom, bathroom, kitchen, and where equipment will go.
Report within 24 hours
The Discharge Home Readiness Report is posted to the portal, followed by a call to go over every Red and Yellow item.
Seven-day check-in
A short call after the patient is home: were the must-do items done, any falls or ER visits, anything new.
Ten areas rated in every report
Getting in, moving through, bed and transfers, bathroom, kitchen and meals, equipment, people and caregiver coverage, supplies and routines, safety and emergencies, and follow-up.
The rating describes the home's readiness for this person. It is not medical clearance. Sessions are not recorded.
