How to refer without sending patient information. Give the patient or family our number, (813) 867-3372, or our booking page. Or call us for a warm handoff. Please do not email or text patient details. With the patient's signed release, we send the report to you through our secure portal.
Physicians and providers
You see the fall risk in the exam room. We see it in the kitchen, the bathroom, and on the entry steps. Our Home Safety Visit includes STEADI-based fall risk screening, a functional assessment in the home, and a prioritized hazard list you can put in the chart.
Our services are private pay. We do not bill Medicare or other insurance, so there is no order or prior authorization to complete.
Good candidates
- A fall or near fall in the past year
- New walker, wheelchair, or oxygen
- Recent hospitalization or rehab stay
- Cognitive changes with a caregiver at home
- Power-dependent equipment before storm season
Physical and occupational therapists
We are therapists too, and we do not replace home health. We cover what visit limits and documentation rules leave out: a full environmental assessment, a written Modification Prescription for contractors, and verification that the work was done to specification.
With the patient's release, we share findings so they support your plan of care rather than compete with it.

Discharge planners and case managers
Ready for Discharge is built for your timeline. We run a three-way video session with the patient in your facility and a family member at the home, then post the Discharge Home Readiness Report within 24 hours.
- One overall rating: Green, Yellow, or Red
- Ten domains, from getting in the door to caregiver coverage
- Red ratings trigger a same-day call to the family, and to you with authorization
- A 7-day check-in call after the patient is home
Facility pricing
$150 per case when the facility pays, or $1,400 for a 10-case pack used within six months.
