This website does not collect medical information
We do not ask for diagnoses, medicines, or health history on this website, in online booking, or by email. Clinical information is collected only in our secure client portal, which operates under a HIPAA Business Associate Agreement.
Booking
Online booking is provided by Calendly. When you book, Calendly collects your name, email, phone number, the ZIP code of the home, and your role, such as family member or discharge planner. Please do not enter health information in booking forms.
Payments
For now, we take card payments by phone. Your card number is entered directly into our card processor during the call. We do not write down, email, or keep your full card number.
Contractor applications
Information contractors submit is used only to evaluate and manage their participation in the network.
Your settings
If you change the text size, your choice is saved in your own browser so the site remembers it. The fall risk quick check is scored in your browser and never sent to us.
Protected health information
We treat all protected health information under HIPAA. We share reports only with people you name on a signed Release Authorization. You may revoke an authorization at any time in writing.
