Ready for Discharge vs Home Safety Visit

Choose Ready for Discharge when someone is coming home from a hospital or rehab stay and you need to know whether the home can support them on day one. Choose the Home Safety Visit when the person is already home and you want to lower their fall risk. Both are $250 in the home or $175 by video.
